FO1 also has this horrid issue where if one of your companions is in a doorway - you're pretty much f'd until they decide to move or you go to another location so they follow you. An issue fixed in FO2 with the 'gtfo of my way' pointer option.
Ian is legit the worst companion for shooting your character in the back. Tycho would never. and Katja uses melee.
